The following forms need to be filled out to register a player at Polonia:

Polonia Wear 2008 - 2009
Return these forms along with the following items to your team manager in order to secure your child's spot on his/her team's roster:
- All forms completely filled out
- Total payment for the 2008-2009 season
- Current picture of the player (small school size photo)
- Player pass or original birth certificate
- Player's signature on a small piece of white paper for the player pass.
- $75.00 check made out to Polonia Youth Soccer. If the child is selected and accepted to play on the Polonia team, the $75 will be credited toward the Polonia fees. If the child does not accept or does not make the Polonia team, then the $75 fee will be returned to the family.
By registering your child, you are commiting to volunteering at the 2008 Polonia Youth Soccer Tournament. Please note that there are no refunds.
Due to club, state, league, coaching, and team commitments once a player has accepted a position on a Polonia team the will be NO REFUNDS.
